Step 1: Checking for eligibility
Eligibility Criteria:
· Person has intellectual disability · Age 16 – 55
Step 2: Fill up and send in application form

Step 3: Initial Interview
Upon receipt of the application form, our Social Worker will arrange to have an initial interview. The person with disability has to be accompanied by a caregiver during the interview which will last about an hour.
Documents to bring include:
For Medical assessment:
Medical certificates/ reports, doctor’s notes, medical appointment cards of application as well as medication (if any). This will help us ascertain the medical needs of the applicant should he be admitted in the DAC. For Social background and Fee Assessment:
Pay slips / proof of income of parents, children, as well as siblings who stay in the same household.
Bankbook of joint or single account under client to be admitted.
Any documentary proof that any family member requiring long term medical care or any such documents that demonstrates a significant drain on the family’s finances. This will help us determine the amount of fees to be charged and any financial assistance to be rendered if necessary.
Step 4: Pre Admission Medical Screening
Applicant’s family will be notified by phone if the pre admission assessment is successful and the negotiated fee to be charged. Applicant will then need to undergo a medical screening and an endorsement by a medical practitioner to certify that applicant does not have any significant illness and is fit to be admitted.
Step 5: Trial Admission
Applicants who successfully pass the interview and medical screening will then undergo a trial admission of period of up to one month. The period of trial may be extended as deemed necessary by the management committee of Bishan Home.
Applicant will be confirmed for admission if he or she passes the trial.
